Registration

 Required forms for members

All students need to fill out a Registration Packet, either online or printed. (Online is preferred, but you can print the packet, fill it out, and bring it with you to rehearsal. Blank packets will also be available at the first rehearsal.) The packet includes five forms: Registration, Emergency Care Authorization*, Hold Harmless*, Rules and Responsibilities, and School Music Program Participation.

*IMPORTANT: All students must have the Emergency Care Authorization and Hold Harmless forms on file, signed by parents and students, in order to participate in rehearsal. Students without these forms will not be allowed to stay at rehearsal. Forms can be filled out in advance and brought with the student, or parents can fill out the forms in person at rehearsal..
